تحلیل روایی و پایایی دستگاه طراحی و ساخته شده برای سنجش دقت حس عمقی مچ پا در افراد مبتلا به پارکینسون

چکیده
حس عمقی یکی از سه منبع بازخوردهای حسی برای حفظ تعادل در انسان است. این حس به دلایلی از جمله کهولت سن و بیماری‌هایی نظیر پارکینسون و دیابت و یا آسیب‌های ورزشی مختل می‌شود. برای کمی‌سازی دقت حس عمقی به طور محدودی از دستگاه AMEDA در مطالعات بین‌المللی استفاده شده اما هیچ نمونه­ای در ایران موجود نبوده است. هدف این مطالعه، ارزیابی پایایی و روایی دستگاه طراحی و ساخته شده‌ی سنجش دقت حس عمقی است. در این راستا 17 فرد مبتلا به پارکینسون و 15 فرد سالم برای شرکت در این مطالعه داوطلب شده‌اند. دقت حس حرکت در مچ پای غالب داوطلبان توسط دستگاه اندازه‌گیری شده است. از متغیرهای ضریب همبستگی درون­طبقه­ای (ICC) و سطح زیر منحنی مشخصه‌ی عملیاتی گیرنده (AUC of ROC) به ترتیب برای تعیین پایایی و روایی دستگاه استفاده شده است. مقدار ICC برای دو جهت حرکت دورسی و پلانتارفلکشن در افراد سالم 80/0 و 87/0 و در افراد مبتلا به پارکینسون 97/0 و 78/0 به دست آمده است. مقدار AUC نتایج در تمایز بین دو گروه سالم و پارکینسون در جهت دورسی فلکشن 94/0 و در جهت پلانتار فلکشن 92/0 است. نتایج نشان داده که دستگاه از پایایی خوب و روایی عالی برخوردار است. از این دستگاه می‌توان برای سنجش دقت حس حرکت در حس عمقی در مطالعات آتی استفاده کرد.

Validity and Reliability Analysis of a Designed and Manufactured Device for Ankle Proprioception Acuity Measurement in People with Parkinson’s

Proprioception, one of the three sources of sensory feedback for maintaining balance in humans, can be disrupted due to factors such as aging, Parkinson's, diabetes, and sports injuries. The AMEDA device, commonly used in international research, has not been designed or utilized in Iran. The purpose of this study is to assess the reliability and validity of a newly developed proprioceptive acuity measuring device. Seventeen individuals with Parkinson's and 15 healthy individuals participated in the study. The device assessed the acuity of sense of motion in the dominant ankle of the participants. The intraclass correlation coefficient (ICC) and the area under the receiver operating characteristic curve (AUC of ROC) were used to determine the reliability and validity of the device. The ICC values for dorsiflexion and plantarflexion were 0.80 and 0.87 in healthy subjects, and 0.97 and 0.78 in people with Parkinson's. The AUC value for differentiating between healthy and Parkinson's groups was 0.94 for dorsiflexion and 0.92 for plantarflexion. The findings indicate that the device exhibits good reliability and validity, and can be utilized in future studies to measure sense of motion acuity in proprioception.
